Q: How do you help patients safely manage pain after surgery?
A: We take a careful and respectful approach to post-surgical comfort care for our patients.
For example, using local anesthetic prior to the procedure and pain pumps for post-procedure comfort allows for a more comfortable 鈥渨ake-up鈥 from general anesthesia.
These measures give our patients the ability to use the lowest effective dosage of opioids, if they choose to use them, for comfort during post-surgical healing. We take the opioid epidemic seriously and follow state guidelines by reviewing a patient鈥檚 medication history before prescribing an opioid. Patients can also manage their pain with nonopioid medications.
We provide post-surgery instructions and we鈥檙e available to answer any questions to ensure our patients鈥 comfort, safety, and well-being during their recovery.
Q: I鈥檓 extremely afraid of going to the dentist, but I need dental work 鈥 and I hate my CPAP. What are my best options?
础:听First of all, with sleep dentistry you can put aside your fear of the dentist. Dr. Dolores Baran offers a range of sedation methods so you can be totally relaxed and have most or all of your dental work completed in one visit.
鈥淚n our office we provide IV sedation, where the patient is in a twilight sleep 鈥 they don鈥檛 feel any pain and they don鈥檛 remember the experience,鈥 she says. 鈥淎nother option is nitrous oxide, or laughing gas, a minimal sedation that makes you feel calm and comfortable while awake. Th ere are also other methods for relaxation that are discussed and tailored to a patient鈥檚 specific needs.鈥
Dr. Baran and her team provide all phases of dentistry and use advanced technologies like the CEREC CAD/ CAM machine, which creates crowns and caps in a single office visit without messy impressions or painful temporaries. Dr. Baran is also the Face of Fastbraces, and is able to straighten teeth in almost half the time required with traditional braces. From fillings and root canals to crowns, gum treatments, and implants, patients can have everything done here while they鈥檙e asleep,鈥 she says.
Dr. Baran is helping patients get a good night鈥檚 sleep with the Oral Sleep Appliance, which in many cases can replace the use of the CPAP (continuous positive airway pressure) machine. Many people don鈥檛 wear their CPAP at night because it鈥檚 annoying and uncomfortable to wear, but sleep apnea can be life-threatening. Dr. Baran can perform an evaluation to see if you鈥檙e a candidate for the Oral Sleep Appliance, which can also be used to control snoring.
Call for a consultation. Get a good night鈥檚 sleep, and wake up with a new smile!
Q: My back always hurts, and pain medication only helps temporarily. Can physical therapy help fix my back?
A: Increasing numbers of patients suffer from chronic pain lasting weeks or months, which can be debilitating. The Pilates Fitness & Physical Therapy Center successfully treats many patients with chronic pain. The difference? A Pilates-based physical therapy program.
Why use Pilates for chronic pain? Pilates helps to strengthen core-stabilizing muscles, and patients benefit from addressing muscle imbalances and postural asymmetries 鈥 thus decreasing wear and tear on the joints. Owner and physical therapist Ron Jegadeesh says: 鈥淪tarting with the first visit, I evaluate postural alignment and teach proper breathing techniques. Then the patient progresses to gentle exercises to encourage pelvic and spine stabilization. Most patients notice a difference after the first few sessions.鈥
Pilates can be specifically tailored to meet the needs of a patient with movement restrictions and varying pain levels. A Pilates Fitness & Physical Therapy Center trainer will design a program to strengthen the deep core muscles, optimizing pelvic and shoulder stabilization, and will use gentle stretching to lengthen tight muscles. Patients feel taller and stronger, and their pain levels dramatically decrease.
Q: I鈥檝e had chronic pain for some time. Is there a program that can get me back to my normal activities without using pain medication?
础:听For people with long-term pain from an accident, injury, or a condition like arthritis, Team Rehabilitation鈥檚 Functional Recovery Program offers a tailored, interdisciplinary approach that restores everyday function and offers a better quality of life without a reliance on opioids.
鈥淓very patient is given a customized program of exercises, cardiovascular workouts, and a stretching routine that incorporates balance and strength,鈥 says occupational therapist Mary Pietrzyk, OTRL. 鈥淧atients also do weight training for both the upper and lower body. We concentrate on the total person, to improve their strength from head to toe.鈥
Physical therapist Shekufa Irani, DPT, says many patients who come to this specialized clinic are using canes, crutches, or walkers to help manage their pain. 鈥淲e work to wean them off of those devices and normalize their gait. Our goal is that they won鈥檛 walk with a limp or need any assistive devices when they finish the program.鈥
Psychologist Olga Slavin-Spenny, Ph.D., presents educational lectures for patients on topics that include stress management, mindfulness, relaxation, and other coping skills. 鈥淭he lectures and exercises are done in group settings,鈥 she says. 鈥淧atients have an opportunity to talk with other people who have chronic pain and who understand what they鈥檙e going through. We have a very positive and supportive environment.鈥

Q: What are the potential long-term consequences of auto accident injuries, and can chiropractic alignment help avoid these problems?
A:听When someone is involved in an auto accident, vertebrae shift out of place. This affects the discs, ligaments, and muscles, and can lead to sprain or strain injuries, or pinched nerves. Pain may not show up for years after the accident and can come in the form of muscle spasms, tingling and numbness in the lower and upper extremities, arthritis in the joints, and other chronic pain issues.
After an accident, it鈥檚 important to have an alignment of the spine to take pressure off the nerves, in order to help alleviate pain and prevent chronic pain in the future. While medications may temporarily relieve pain, chiropractic adjustments can correct the problem that causes your pain. If you鈥檝e been involved in an auto accident, call us and we鈥檒l be there for you.

Q: Are there ways to overcome the fear of going to the dentist?
础:听The fear of pain associated with dental procedures keeps many people from living life to its fullest, yet it doesn鈥檛 have to be that way. With conscious sedation, dental work is completed while the patient is sleeping. IV sedation minimizes anxiety, and allows work to be performed without the patient feeling any anxiety or discomfort.
For patients with gum disease or gum recession, conscious sedation and minimally invasive technology have had transformative effects. Using the pinhole technique, treating gum recession is far more comfortable and predictable than in the past. The procedure eliminates the need to cut tissue from the palate or other areas to cover receding gums. Instead, a small pinhole is made in the gum tissue above or below the receded roots of the teeth, and special instruments are used to loosen and bring healthy gum tissue over the receded tooth roots, improving the appearance of the teeth and eliminating sensitivity and discomfort. Material is then placed through the pinhole, to thicken and stabilize the tissue that鈥檚 in place. The pinhole technique is designed to be scalpel-free and suture-free.
Dr. Joseph R. Nemeth also performs LANAP, a laser procedure for treating gum disease that eradicates bacteria under the gums and permits new bone growth and gum reattachment. The benefits of healthier gums can reach far beyond oral hygiene 鈥 studies link gum disease to many other serious illnesses including heart disease, diabetes, Alzheimer鈥檚 disease, and certain cancers, so it鈥檚 crucial to attend to your dental health. There鈥檚 no need to fear the dentist, because conscious sedation and noninvasive procedures have made pain-free dental visits a reality.
Q: I have joint pain. What nonsurgical, drug-free therapies can help me?
A: Stem cells are specialized cells that seek out inflammation and degeneration in the body and then work to regenerate damaged tissue. Although stem cells rapidly decline with age, advanced treatments using human umbilical cord stem cells have been shown to help repair and regenerate muscle, bone, cartilage, and tendons. FDA-cleared stem cell injections and infusions are used to treat joint damage and pain, injuries, and a wide variety of other health complications.
Platelet-rich plasma (PrP) therapy uses blood taken from your arm, which is then processed to concentrate the growth factor-enriched platelets. This solution is injected into damaged tissues to stimulate a natural healing response and promote tissue rejuvenation. Under ultrasound guidance, PrP therapy is used for a variety of joint conditions to repair cartilage, reduce acute and chronic pain, speed recovery, and improve function.
Q: How do you know if puffy, under-eye bags are caused by fatigue, allergies, or sinus problems?
础:听Typically, these conditions are unrelated, says ophthalmic and facial plastic surgeon Dr. Evan Black. 鈥淚f you鈥檝e had a good night鈥檚 sleep and you don鈥檛 have allergies or sinus disease, or these conditions have been treated, then the bags you see are what鈥檚 called herniated orbital fat, and that鈥檚 the most common reason that people have the appearance of bags under their eyes,鈥 he says. 鈥淭his is based on the fact that the eye sits in a space called the orbit, which has fat compartments that can protrude under the eye.鈥
To improve the appearance of baggy lower eyelids, the fat can be removed, repositioned, and/or sculpted. Minimally invasive lower eyelid surgery is a no-stitch 鈥渂ag鈥 removal that dramatically improves the appearance of the area around the eyes.
鈥淚t鈥檚 a 30- to 40-minute procedure with local anesthesia and sedation for comfort. Since we鈥檙e not cutting or removing any skin, surgery risks are significantly reduced,鈥 Dr. Black explains. 鈥淲e also use a fractional CO2 laser to diminish wrinkles, improve tone, and tighten up the delicate skin in the lower eyelids. This outpatient surgery is a permanent solution that can turn back the clock and help you maintain a natural, youthful look.鈥

Q: How are TMJ disorders treated?
础:听Each case is unique and different. People with TMJ (TMD) experience varying degrees of discomfort and pain. These can be caused by trauma, occlusal (bite) disorders, deep pain, parafunction, or emotional stress. The extent of the problem differs in every patient and must be treated on an individual basis.
The TMJ patients we treat are primarily associated with bite disorders and pain. Typically, these patients have sought out other treatment that has failed. The treatment plan for each patient is unique and, to a great degree, dependent on the tolerance for their discrepancies and pain. Treatment ranges from simple appliances to very precise ones for severe cases. In rare cases, more than one appliance is needed. All symptoms must be stabilized for a period of time before any further treatment can be initiated.
As a consumer, it鈥檚 important to know the qualifications, skill level, and success rate of the person treating you for TMJ. They should be able to explain what the problems are and how they will be treated. If it doesn鈥檛 make sense, or you鈥檙e unsure, get a second opinion!
